The first part of this course reviews the general principles of incident investigations, including the importance of identifying underlying causes of the incident as well as immediate causes, the components of an investigation report, and following up on corrective actions that are taken as a result of the report’s recommendations.

The rest of the course is an intensive, workshop-style review of the investigation process that revolves around a video, The Sudden Death of William Powell. Participants play the role of incident investigators as they watch the video, take notes of the testimony in order to identify the immediate and underlying causes of the incident and come up with recommendations on how to prevent the incident from ever happening again.
